ATTENTION: BEGINNING 8/5/19 THE ARIZONA WOTC UNIT WILL BE PROCESSING APPLICATIONS WHICH DO NOT CONTAIN THE NECESSARY DOCUMENTATION FOR MAKING A DETERMINATION, I.E., FELON AND OR VETERAN TARGET GROUPS, IN CONJUNCTION WITH THE DOL RECOMMENDATIONS, IN THE FOLLOWING MANNER. A NOTIFICATION LETTER WILL BE SUBMITTED TO APPROPRIATE EMPLOYER/CONSULTANT INDICATING THEY HAVE 90 DAYS FROM THE DATE OF THAT LETTER TO UPLOAD THE REQUIRED VERIFICATION DOCUMENTS. THE APPLICATION WILL BE LEFT IN A PENDING STATUS FOR NO MORE THAN 90 DAYS TO ACCOMMODATE UPLOADING THE REQUIRED VERIFICATION DOCUMENTS.
By completing this process, you are requesting an account to access the automated WOTC System. You are considered to be the legal forms holder of original signature documents. A legal holder must be designated to hold (maintain) original signature documents (IRS 8850, ETA 9061 and any supporting documentation) for a period of five years from the year that the tax credit is filed by the employer. When your company is designated as the legal forms holder, you are authorized to enter data from original signature documents and submit this information electronically to the AZ WOTC Database no later than 28 business days after the employee's Start-to-Work date. The electronic submission date for each application serves as its postmark date.
Power of Attorney (POA) for your applications must be in the Arizona WOTC database with current effective dates before entering/ submitting your on-line applications. Please do not mail Applications which do not have a current POA in place to the AZ State Workforce Agency.

2015-11-18 The office of Management and Budget (OMB) has officially approved the Department of Labor's (DOLs) request to extend WOTC forms (such as the ETA 9061, 9062, an 9063) without revisions for three years (2018-08-31).
2015-12-18 The PATH Act reauthorized the WOTC program retroactively for all current target groups from 2014-12-31 to 2019-12-31. It also introduced a new Qualified Long-term Unemployment Recipient target group authorized after 2015-12-31.
